This form is designed to lodge a complaint where there are allegations of breaches of policies under the National Integrity Framework. If you wish to remain anonymous, you will need to lodge a report with Sport Integrity Australia on 1300 027 232. 

Prior to completing the form, Boccia Australia recommends you familiarise yourself the Complaints Policy and the Flowchart. Please note this form may take around 30 - 40 minutes to fill out if you have a lot of information to provide, so it is recommended you find a time and place you can be uninterrupted whilst completing the form. Unfortunately, the form does not allow for auto-saving, so should be completed in one sitting.

If your concern relates to an emergency, or a situation where a person's safety is at immediate risk of harm, call the Police on 000 prior to continuing with this form. If you believe a criminal offence may have occurred, please contact your local police prior to continuing with this form.
